The Canadian Patient Safety Institute’s (CPSI) Medication Safety at Care Transitions Safety Improvement Project is a learning collaborative to support frail elderly patients with better health outcomes, reduce length of stay, fewer hospital readmissions, and overall cost savings to the healthcare system. As part of CPSI’s project, eight teams of four members will be selected to attend in-person and virtual learning sessions and be supported by expert faculty and coaches from across the country. The teams will also receive content on implementation and knowledge translation to assist in implementation efforts to ensure project gains. CPSI will be holding a webinar on February 5th or 12th.
