Essex-Windsor EMS launched Vulnerable Patient Navigator program (VPN) 2016 to curb the number of frequent, non-emergency, 911 callers. As part of the program, low-priority calls are connected with a paramedic who helps them navigate the healthcare system to determine which services they may need. Once these patients are enrolled in VPN, there's a needs assessment that includes home visits to better equip patients to manage some of their health issues. The program found success among patients enrolled, but overall, the number of 911 callers went up instead. Essex-Windsor EMS believes these results, including the high patient satisfaction rates, should be interpreted as proof of an increased need for the VPN.
