HealthLandscape is a U.S. company based on an interactive web atlas. It belongs to the American Academy of Family Physicians. It launched an atlas called the Community Vital Signs Population Health Profiler tool, which gives physicians free data on the social determinants of health in the communities they serve. It lets users create detailed ZIP code area maps of their service area and key community measures, including the distribution of age, ethnicity, social deprivation, inequities, access to healthcare and chronic conditions. Specifically, the chronic conditions report shows percentages of the population that are diabetic, obese, hypertensive or uninsured. As for a social inequality report, it includes metrics such as income inequality, linguistic isolation and race differences in poverty and disability.
