The Diabetes Self-Management Health Coaching Program connects participants with a volunteer health coach once a week by telephone for three months. Participants work with their assigned health coach to learn and develop skills to manage their diabetes. Coaches have lived experience in managing chronic conditions but aren’t medically trained and don’t provide medical advice. The weekly coaching support isn't meant to replace that of a family doctor but it's offered in addition to recommendations participants may receive from their healthcare provider.
