Canadian Foundation for Healthcare Improvement expands access to EPAC program that aims to improve end-of-life care

The CFHI will be working with seven additional organizations to spread Embedding Palliative Approaches to Care (EPAC). EPAC helps staff in long term care to identify residents who could benefit from a palliative approach to care, to have conversations with them and their families about what they want, and to develop care plans. EPAC is already in 48 long term care homes. From 2014- 2018, the CFHI says the program resulted in a 56% decrease in ED transfers and a 45% reduction in number of days residents spent in acute care in the last three months of their life. CFHI will provide funding to support the following organizations to spread the approach to 22 homes in this initial expansion:

  1. Yukon Health and Social Services;
  2. Institute for Continuing Care Education and Research (ICCER), Alberta;
  3. Parkwood Seniors Community, Waterloo, Ontario;
  4. Haliburton Highlands Health Services, Haliburton, Ontario;
  5. Centre intégré universitaire de santé et de services sociaux (CIUSSS) de la Mauricie-et-du-Centre-du-Québec, Quebec;
  6. New Brunswick Association of Nursing Homes; and
  7. Western Health, Newfoundland and Labrador.
