Study finds N.S. community pharmacies committed 100K errors in 7 years

Research led by the University of Toronto found 98,097 errors at 301 community pharmacies in Nova Scotia between 2010 and 2017. Of those, 928 resulted in patient harm. In its analysis of the data, the Nova Scotia College of Pharmacists says that in 82% of cases, the errors were detected before the medications were dispensed to patients. Of the errors recorded, 30% related to incorrect dosages or frequencies, 20% dealt with incorrect strengths or concentrations and a further 20% involved incorrect drugs.
